Comprehensive Trip Log Form Definitions

The following definitions apply specifically to the Comprehensive Trip Log (CTL) form (CalRecycle 203). For a broader list of definitions for the manifest program, see the glossary.

Address, City, State and Zip: Business address of the Hauler, Tire Dealer, Generator or End Use Facility operator.

Address Same As Hauler: This is the address of the hauling business and can be used when the hauler is either shipping tires to, or removing tires from, this location.

Amount (AMT): This is the quantity of tires being picked up or delivered. The spaces following the decimal sign are to be used only in reporting tenths or hundredths of a ton.

Business Name: The business name of the Hauler, Generator, or End Use Facility operator.

Business Phone: The phone number of Hauler, Generator, or End Use Facility operator.

Common Carrier: Means a person or corporation with a land based motor vehicle transportation business that offers service to the general public. The common carrier can only haul waste or used tires when they hauls a commodity other than used or waste tires to an original destination point and then transports used or waste tires on the return part of the trip. The revenue derived from transporting the used or waste tires must be incidental when compared to the revenue earned by the common carrier. The box inside the address area must be checked if this is a common carrier business.

Confidentiality Claim: Information on the comprehensive trip log that can be claimed confidential are “load type” and “load amount” and the specific transaction between the generator and hauler (pick up) and between the hauler and end use facility (delivery) for a given load. “

Date: The date is when the tire transaction took place.

Decal Number: Unique identification number assigned by the CalRecycle to each registered waste tire hauling vehicle. The decal is located on the lower right hand corner of the windshield (passenger side).

Delivery: Indicate if the information on the comprehensive trip log receipt relates to a tire delivery.

Driver’s Name: The name of the driver picking up or delivering tires.

Driver’s Signature and Date: Signature and date of the driver picking up or delivering tires.

License Plate Number: The license plate number of the vehicle picking up or delivering tires and the state of issuance.

Load Date: Date the tires were picked up from, or delivered to the Generator or End Use Facility.

Load Amount: The number that represents the volume of tires picked up based on the load type indicated (whole tire count volume in cubic yards weight in pounds or weight in tons.)

Load Type: The measurement used to determine the amount of tires picked-up or delivered.

Manifest Number: Each comprehensive trip log contains a unique preprinted number. The comprehensive trip log is located in upper right hand corner of the document. This number helps to track individual pick-ups and deliveries and is also printed on the 3 corresponding receipts form the document.

Pick Up: Indicate if the information on the comprehensive trip log receipt relates to a tire pickup.

Representative Name: The printed name of the person representing the Tire Dealer, Generator, or End Use Facility business where tires are being picked up or delivered.

Representative Signature and Date: Signature and date of the Generator or End Use Facility operator authorized to dispose of or accept used/waste tires.

Tire Program ID: Tire program identification (TPID) numbers are unique numbers assigned by CalRecycle to each business or public entity involved in the generation, hauling and delivery or disposal of waste tires.

Volume Cubic Yards: The volume representation of the tire counts to be in cubic yards.

Weight in Pounds: The volume representation of the tire counts to be in pounds.

Weight in Tons: The volume representation of the tire counts to be in tons.

Whole Tire Count: The volume representation of the tire counts to be in whole tires.
